Summary

NSAIDs such as parecoxib can reduce the blood pressure lowering effect of antihypertensives through sodium and water retention and reduced vasodilatory prostaglandins. The effect with alpha-blockers is usually smaller than with ACE inhibitors or diuretics.

What this interaction means in plain words

Parecoxib and Prazosin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Inhibition of prostaglandin synthesis leading to fluid retention and vasoconstriction, partly opposing antihypertensive action.

What to do

Check blood pressure if parecoxib is used for more than a short period. Adjust antihypertensive therapy if control worsens.

What to ask your pharmacist
