Summary

Blocking beta-receptors with pindolol leaves the alpha-mediated vasoconstriction of phenylephrine unopposed, and the reflex slowing of heart rate may be exaggerated. This can produce a marked rise in blood pressure with reflex bradycardia, although it is uncommon at usual doses.

What this interaction means in plain words

Phenylephrine and Pindolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Beta-blockade removes beta2-mediated vasodilation, leaving unopposed alpha-adrenergic vasoconstriction.

What to do

Monitor blood pressure and heart rate when phenylephrine is given systemically to patients on pindolol. Use the lowest effective phenylephrine dose, and consider alternatives to oral decongestants in hypertensive patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
