Summary

Nonselective beta-blockade leaves alpha-1 mediated vasoconstriction unopposed and blunts reflex tachycardia, so phenylephrine can cause a marked rise in blood pressure and reflex bradycardia. Hypertensive episodes are uncommon but can be significant.

What this interaction means in plain words

Phenylephrine and Propranolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Phenylephrine belongs to Respiratory; Propranolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Nonselective beta-blockade prevents beta-2 vasodilation and cardiac compensation, unmasking alpha-1 pressor effects.

What to do

Monitor blood pressure and heart rate when systemic phenylephrine is used with propranolol. Use the lowest effective phenylephrine dose and avoid oral decongestants in hypertensive patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
