Summary

Both drugs have sympathomimetic activity, so using them together can add to increases in heart rate and blood pressure and may provoke palpitations, tremor or anxiety. Patients with cardiovascular disease or arrhythmia are most at risk.

What this interaction means in plain words

Pirbuterol and Yohimbine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive adrenergic stimulation: pirbuterol is a beta2-agonist and yohimbine is an alpha2-antagonist that raises noradrenergic outflow.

What to do

Monitor heart rate, blood pressure and symptoms of adrenergic excess when the two are used together. Avoid yohimbine in patients with significant cardiovascular disease or uncontrolled hypertension.

What to ask your pharmacist
