Summary

Pralidoxime reactivates acetylcholinesterase, whereas pyridostigmine inhibits it, so their effects are opposed. Pyridostigmine is also used as a pretreatment for nerve agent exposure alongside oxime therapy, and in myasthenia patients pralidoxime at high doses may itself cause neuromuscular weakness.

What this interaction means in plain words

Pralidoxime and Pyridostigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Opposing effects on acetylcholinesterase activity: reversible inhibition versus enzyme reactivation.

What to do

Monitor neuromuscular function and respiratory status when pralidoxime is given to a patient on pyridostigmine, and adjust therapy by clinical response.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
