Summary

Pramlintide slows gastric emptying and intestinal transit, and trospium is an antimuscarinic that also reduces gastrointestinal motility. Together they can cause marked delay in gastric emptying, constipation, and in severe cases ileus or obstruction. Pramlintide labeling advises against use with drugs that slow GI motility.

What this interaction means in plain words

Pramlintide and Trospium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Pramlintide belongs to Endocrine; Trospium belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic inhibition of gastrointestinal motility by an amylin analog and an anticholinergic.

What to do

Avoid the combination where possible, or monitor for constipation, abdominal distension and bloating. Consider alternatives to the antimuscarinic and stop pramlintide if significant GI hypomotility develops.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
