Summary

Rapacuronium is a nondepolarizing neuromuscular blocker, and pyridostigmine antagonizes it by raising acetylcholine at the junction. Higher doses of the blocker may be needed, and in myasthenia gravis patients sensitivity to nondepolarizing blockers is increased. Rapacuronium has been withdrawn from most markets.

What this interaction means in plain words

Pyridostigmine and Rapacuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Cholinesterase inhibition increases acetylcholine, competitively opposing nondepolarizing neuromuscular blockade.

What to do

Monitor neuromuscular blockade with a nerve stimulator and titrate the blocker accordingly.
