Summary
In myasthenia gravis, starting corticosteroids can cause a temporary worsening of muscle weakness, and this may be more pronounced in patients taking anticholinesterase drugs such as pyridostigmine. Severe weakness, including respiratory compromise, has been described.
What this interaction means in plain words
Pyridostigmine and Triamcinolone acetonide injectable can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Corticosteroids can transiently worsen neuromuscular transmission in myasthenia gravis, and an additive effect with anticholinesterase therapy is proposed but not fully established.
What to do
Monitor myasthenic patients closely for weakness and respiratory function after steroid initiation, ideally in a setting where ventilatory support is available. Consider adjusting the pyridostigmine dose, and start steroids at low doses with gradual titration.
