Summary

Salsalate is a salicylate NSAID that can blunt the antihypertensive effect of telmisartan. In older or volume-depleted patients the combination can impair renal function and raise potassium.

What this interaction means in plain words

Salsalate and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of prostaglandin synthesis reduces renal vasodilation and sodium excretion, opposing angiotensin receptor blockade and lowering glomerular perfusion.

What to do

Monitor blood pressure, serum creatinine and potassium, and keep the patient well hydrated. Use the lowest effective salsalate dose for the shortest time or choose paracetamol.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
