خلاصه
Intravascular iodinated contrast can cause acute kidney injury, which reduces metformin clearance and may lead to metformin accumulation and lactic acidosis. The risk is greatest with impaired renal function or intra-arterial administration.
معنای این تداخل به زبان ساده
Diatrizoate meglumine / diatrizoate and Metformin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
چرا رخ میدهد
Contrast-induced decline in renal function reduces metformin elimination, leading to accumulation.
اقدام لازم
In patients with eGFR below 30, or with acute kidney injury or intra-arterial contrast, withhold metformin at the time of the procedure and for 48 hours afterward. Restart only after renal function has been re-evaluated and found stable.
