Indications

Mécanisme d'action

HIV-1 protease inhibitor; antiviral activity of the lopinavir/ritonavir combination is due to lopinavir, while ritonavir inhibits CYP3A metabolism of lopinavir and raises its plasma levels.

Posologie adulte habituelle (résumé de la notice)

HIV-1 infection: tablets 400/100 mg (two 200/50 mg tablets) twice daily, or oral solution 400/100 mg (5 mL) twice daily, with other antiretrovirals

Résumé de la notice pour la première ligne de posologie uniquement ; les sections complètes sur la posologie figurent dans l'application.

Grossesse et allaitement

Grossesse
Safe Antiretroviral pregnancy registry data show no rise in birth defects. In pregnancy give 400/100 mg twice daily (once-daily dosing is not recommended) and avoid the oral solution, which contains about 42% alcohol and propylene glycol. Can be part of HIV treatment.
Allaitement
Safe Low levels pass into milk and reach some infants' blood; no infant harm clearly caused. US label: do not breastfeed (HIV transmission, resistance). Current US guidance supports breastfeeding only with sustained viral suppression after counselling; otherwise avoid.

Effets indésirables signalés

  • Diarrhea
  • Nausea
  • Vomiting
  • Abdominal pain
  • Headache
  • Asthenia

Contre-indications

  • Hypersensitivity to any ingredient, including ritonavir (e.g., toxic epidermal necrolysis, Stevens-Johnson syndrome, erythema multiforme).
  • Co-administration with drugs highly dependent on CYP3A for clearance where elevated levels cause serious or life-threatening reactions.
  • Co-administration with potent CYP3A inducers (loss of virologic response); Table 3 lists alfuzosin, rifampin, ergot derivatives, cisapride, St. John's Wort, lovastatin, simvastatin, sildenafil (Revatio), pimozide, triazolam, oral midazolam.

Précautions particulières

  • CYP3A inhibition can raise levels of concomitant drugs, causing severe or fatal events; review all medications.
  • Pancreatitis, including fatalities; suspend therapy as clinically appropriate if suspected.
  • Hepatotoxicity: monitor liver function, especially with underlying hepatitis B or C or cirrhosis.

Informez votre pharmacien ou votre médecin en cas de grossesse ou d'allaitement, de maladie rénale ou hépatique, d'allergies, ainsi que de tout autre médicament ou complément que vous prenez.
