Résumé

Lopinavir, boosted with ritonavir, markedly raises ergotamine exposure. This can cause ergotism with severe peripheral and cerebral vasospasm, leading to ischemia of the limbs, bowel or brain.

Ergotamine and Lopinavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Do not co-administer; use a non-ergot acute migraine therapy such as a triptan-free alternative appropriate for the patient while on lopinavir/ritonavir. Seek urgent care if numbness, cold extremities or severe pain develop.
