Indications
Mécanisme d'action
Injectable cyclohexapeptide somatostatin analogue that binds and activates somatostatin receptors (notably SSTR5, over-expressed in corticotroph tumors), inhibiting ACTH secretion and thereby decreasing cortisol secretion.
Posologie adulte habituelle (résumé de la notice)
Cushing's disease: initial 0.6 mg or 0.9 mg subcutaneously twice a day; range 0.3 mg to 0.9 mg twice a day; titrate to response and tolerability
Grossesse et allaitement
- Grossesse
- Unknown Limited human data. In rabbits it caused developmental delay at doses below human exposure without maternal toxicity; in rats, malrotated limbs at very high doses. EU label: not recommended in pregnancy; women may become fertile again as cortisol or GH falls, so use contraception.
- Allaitement
- Unknown No human milk data; passes into rat milk. EU label: stop breastfeeding during treatment; US label: weigh benefits. LactMed: a large peptide, likely poorly excreted and digested by the infant, but an alternate drug is preferred.

- Nausea
- Hyperglycemia

Contre-indications

Précautions particulières
- Hypocortisolism may occur; monitor, and consider dose reduction or interruption and temporary glucocorticoid replacement.
- Hyperglycemia and diabetes: monitor glycemia weekly for first two to three months and after dose increases; optimize antidiabetic therapy before starting.
- Bradycardia and QT prolongation: use caution in at-risk patients; baseline ECG recommended; correct hypokalemia and hypomagnesemia.
