Mécanisme d'action
Selumetinib inhibits mitogen-activated protein kinase kinases 1 and 2 (MEK1/2), upstream regulators of the ERK pathway. In NF1 mouse models it inhibited ERK phosphorylation and reduced neurofibroma numbers, volume and proliferation.
Posologie adulte habituelle (résumé de la notice)
NF1 with symptomatic, inoperable plexiform neurofibromas (capsules): 25 mg/m2 orally twice daily, swallowed whole, with or without food, by body surface area; until disease progression or unacceptable toxicity
Grossesse et allaitement

- Unsafe Can cause fetal harm: in mice it caused embryo death, structural defects and lower fetal weight at over 5 times human exposure. No human data. Check pregnancy status; use effective contraception during and for 1 week after; it may weaken hormonal pills, so add a barrier method.
- Allaitement
- Unsafe No human milk data; it and its active metabolite reach mouse milk. LactMed: 98% protein bound, so milk levels are likely low. Because of possible infant toxicity, do not breastfeed during treatment and for 1 week after the last dose.

Contre-indications
- None.
Précautions particulières
- Left ventricular dysfunction: assess ejection fraction before treatment, every 3 months in the first year, then every 6 months.
- Ocular toxicity (RVO, RPED, blurred vision): perform ophthalmic assessments; permanently discontinue for RVO.
- Gastrointestinal toxicity: start anti-diarrheal agent at first loose stool and increase fluids.
