- Grossesse
- Unsafe Contraindicated in pregnancy: in rats it caused craniofacial and skeletal malformations and fetal death at 10 times human exposure (endothelin and angiotensin blockade). Exclude pregnancy before and during treatment; use effective contraception until 1 month (EU) or 2 weeks (US) after stopping.
- Allaitement
- Unknown No human milk data. US label: do not breastfeed (possible infant low blood pressure); EU: should not be used while breastfeeding. LactMed has no data and repeats the maker's advice. Prefer not to breastfeed during treatment.

Mises en garde encadrées
- Hepatotoxicity: available only through the restricted FILSPARI REMS. Aminotransferase elevations of at least 3-times ULN occurred in up to 3.5% of patients. Measure transaminases and bilirubin before treatment and every 3 months; interrupt and monitor if above 3-times ULN. Generally avoid in patients with baseline aminotransferases above 3-times ULN.
Contre-indications
- Pregnancy
- Concomitant use with ARBs, ERAs, or aliskiren
Précautions particulières
- Hepatotoxicity: monitor aminotransferases and bilirubin; stop and seek medical attention if symptoms of liver injury occur.
- Hypotension: consider adjusting other antihypertensives; consider dose reduction or interruption if it develops.
- Acute kidney injury: monitor kidney function periodically; patients with renal artery stenosis, CKD, severe heart failure or volume depletion are at particular risk.
