Mécanisme d'action
Sufentanil is an opioid agonist. In balanced general anesthesia it has been reported to be as much as 10 times as potent as fentanyl.
Posologie adulte habituelle (résumé de la notice)
Analgesic adjunct to general anesthesia: doses up to 8 mcg/kg IV by slow injection or infusion; total dosage of 1 mcg/kg/hr or less recommended; individualize
Grossesse et allaitement

- Unknown No human data on birth defects; no malformations in animals, but embryo loss in rabbits near the human dose. It crosses the placenta: use near delivery can depress the newborn's breathing; longer use can cause neonatal opioid withdrawal. Widely used epidurally in labour; have naloxone ready.
- Allaitement
- Safe Epidural or IV use in labour or briefly after birth transfers little into milk; LactMed: not a reason to stop. Limit postpartum use to a few days, then non-opioid pain relief. US labels: monitor for infant sedation and breathing problems; call a doctor if very sleepy or limp.

Mises en garde encadrées
- Exposes users to addiction, abuse and misuse, which can lead to overdose and death; assess risk and reassess regularly. Life-threatening or fatal respiratory depression may occur, especially at initiation or dose increase. Concomitant benzodiazepines or other CNS depressants may cause profound sedation, respiratory depression, coma and death. CYP3A4 inhibitors may raise sufentanil levels and cause fatal respiratory depression; stopping a CYP3A4 inducer may also raise levels; monitor.

- Hypersensitivity to sufentanil (e.g., anaphylaxis)
Précautions particulières
- Life-threatening respiratory depression; give only by trained persons with resuscitative equipment; respiratory depression may outlast analgesia
- Increased risk in COPD, elderly, cachectic or debilitated patients; monitor closely
- Skeletal muscle rigidity; manage with a neuromuscular blocking agent
