Indications

Mécanisme d'action

Mechanism in tardive dyskinesia and Huntington's chorea is unclear, but thought to be mediated through reversible inhibition of vesicular monoamine transporter 2 (VMAT2), which regulates monoamine uptake from the cytoplasm into synaptic vesicles.

Posologie adulte habituelle (résumé de la notice)

Tardive dyskinesia: initial 40 mg orally once daily; after one week increase to the recommended 80 mg once daily; 40 mg or 60 mg once daily may be considered depending on response and tolerability.

Résumé de la notice pour la première ligne de posologie uniquement ; les sections complètes sur la posologie figurent dans l'application.

Grossesse et allaitement

Grossesse
Unsafe May cause fetal harm; human data insufficient. Animal studies showed increased stillbirths and postnatal pup mortality at doses below the MRHD. Advise pregnant women of potential fetal risk.
Allaitement
Unsafe No human milk data; advise women not to breastfeed during treatment and for 5 days after the final dose.

Effets indésirables signalés

  • Somnolence (somnolence, fatigue, sedation)
  • Anticholinergic effects
  • Balance disorders/fall
  • Headache
  • Akathisia
  • Vomiting

Mises en garde encadrées

  • VMAT2 inhibitors, including valbenazine, can increase the risk of depression and suicidal thoughts and behavior in patients with Huntington's disease. Balance these risks against the clinical need for chorea treatment, monitor closely for emergence or worsening of depression, suicidality or unusual behavior changes, inform patients, caregivers and families to report concerning behaviors promptly, and use particular caution in patients with a history of depression or prior suicide attempts or ideation.

Contre-indications

  • History of hypersensitivity to valbenazine or any components of the product.

Précautions particulières

  • Hypersensitivity reactions including angioedema (involving larynx, glottis, lips, eyelids) can occur and may be fatal; discontinue if they occur.
  • Somnolence and sedation: avoid driving or operating hazardous machinery until effects are known.
  • QT prolongation: may be clinically significant with strong CYP2D6/CYP3A4 inhibitors or in CYP2D6 poor metabolizers; avoid in congenital long QT syndrome or arrhythmias associated with prolonged QT.
