Résumé
Patients on ACE inhibitors undergoing apheresis with a protein A immunoadsorption column have developed anaphylactoid reactions. Bradykinin accumulates because ACE inhibition blocks its breakdown, and column treatment can further activate bradykinin-related pathways.

Enalapril-hydrochlorothiazide and Protein a column are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

ACE inhibitors block kininase II degradation of bradykinin, which accumulates during immunoadsorption and triggers anaphylactoid reactions.
