Résumé
Combining an ACE inhibitor with sacubitril/valsartan markedly raises the risk of angioedema due to reduced bradykinin breakdown. Additive hypotension, hyperkalemia and renal effects from RAS blockade also occur.

Enalapril-hydrochlorothiazide and Sacubitril / valsartan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Concurrent inhibition of neprilysin and ACE reduces bradykinin degradation, causing bradykinin-mediated angioedema.
