Hogyan hat
CNS depressant. Mechanism in narcolepsy is unknown. It is the sodium salt of GHB, a metabolite of GABA; therapeutic effects are hypothesized to be mediated through GABA B actions at noradrenergic, dopaminergic and thalamocortical neurons.
Szokásos felnőttadag (alkalmazási előírás szerinti összefoglaló)
Narcolepsy (cataplexy or EDS): start 4.5 g per night orally in two divided doses (2.25 g at bedtime and 2.25 g 2.5 to 4 hours later); increase by 1.5 g per night at weekly intervals; effective range 6 g to 9 g per night; doses higher than 9 g per night not studied.
Terhesség és szoptatás
- Terhesség
- Unknown No adequate human data. Rat pre/postnatal study showed increased stillbirths and decreased offspring viability and growth at a clinically relevant dose.
- Szoptatás
- Unknown GHB is excreted in human milk; insufficient information on infant risk. Weigh breastfeeding benefits against maternal need.
Jelentett mellékhatások
- Nausea
- Dizziness
- Vomiting
- Somnolence
- Enuresis
- Tremor
Fekete dobozos figyelmeztetések
- CNS depression: Xyrem is a CNS depressant; obtundation and clinically significant respiratory depression occurred in adults at recommended doses.
Ellenjavallatok
- Combination with sedative hypnotics.
- Combination with alcohol.
- Succinic semialdehyde dehydrogenase deficiency.
Kinek kell óvatosnak lennie
- Concurrent use with other CNS depressants may increase risk of respiratory depression, hypotension, profound sedation, syncope and death; consider dose reduction or discontinuation.
- No hazardous activities requiring mental alertness for at least 6 hours after dosing.
- May impair respiratory drive; increased central apneas and desaturation observed in adults and children.
