Both drugs prolong the QT interval, so combination raises the risk of torsades de pointes and sudden cardiac death. Pimozide labeling and citalopram labeling both caution against or contraindicate co-use with QT-prolonging drugs. Risk is greater at higher citalopram doses and in patients with electrolyte disturbance or cardiac disease.

Miért jön létre
Additive QT prolongation. Citalopram is also partly metabolized by CYP2D6 and CYP3A4, and pimozide is a CYP2D6 and CYP3A4 substrate, so pharmacokinetic effects may add to this.
Teendő
Do not co-administer. Choose an antidepressant with less QT effect, such as sertraline, and review the antipsychotic need.
