Összefoglaló
Cocaine blocks reuptake of norepinephrine, dopamine and serotonin, while phenelzine prevents breakdown of these amines. Together they can cause marked sympathetic stimulation with severe hypertension, hyperthermia, arrhythmias and serotonin toxicity. Topical cocaine has limited absorption but the combination is still labeled as contraindicated.

Cocaine and Phenelzine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Avoid cocaine, including topical anesthetic use, in patients taking phenelzine or within 14 days of stopping it; choose a different local anesthetic. If exposure occurs, monitor blood pressure, heart rate and temperature and treat hypertensive crisis or serotonin toxicity urgently.
