Összefoglaló
Iodinated contrast can cause acute kidney injury, which reduces metformin clearance and may lead to metformin accumulation and lactic acidosis. The risk is greatest with impaired renal function or intra-arterial administration.
Mit jelent ez a kölcsönhatás közérthetően
Diatrizoate and Glyburide-metformin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Miért jön létre
Contrast-induced decline in renal function reduces metformin elimination and raises the risk of lactic acidosis.
Teendő
Hold metformin at the time of or before contrast in patients with eGFR below 30, liver disease or intra-arterial studies. Restart after 48 hours only once renal function is confirmed stable.
Mit kérdezzen a gyógyszerészétől
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
