Erythromycin raises dihydroergotamine concentrations, which can cause ergotism with severe peripheral and cerebral vasospasm and ischemia. Cases of limb and organ ischemia have been reported with CYP3A4 inhibitors and ergots.

Dihydroergotamine and Erythromycin lactobionate are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
