Ketoconazole substantially increases eliglustat exposure, which can prolong the PR, QTc and QRS intervals and cause arrhythmias. Labeling contraindicates or restricts the combination depending on CYP2D6 metabolizer status.

Eliglustat and Ketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ketoconazole inhibits CYP3A4 and, to a lesser extent, CYP2D6, the two main pathways for eliglustat clearance.

Avoid in intermediate and poor CYP2D6 metabolizers and when combined with a CYP2D6 inhibitor. In extensive metabolizers, eliglustat dose reduction is required, with ECG monitoring.
