Fosamprenavir (as amprenavir, usually ritonavir-boosted) inhibits CYP3A4 and can substantially increase salmeterol exposure. This raises the risk of QT prolongation, tachycardia and arrhythmia. Systemic fluticasone exposure also rises, risking adrenal suppression.

Avoid concurrent use and switch to an alternative inhaled corticosteroid/LABA less reliant on CYP3A4, such as beclomethasone-based therapy. Monitor ECG and cortisol-related effects if it cannot be avoided.
