Ketoconazole markedly increases sirolimus blood concentrations, causing a risk of immunosuppressant toxicity such as myelosuppression, hyperlipidemia, infection and impaired wound healing. Labeling advises against the combination.

Ketoconazole and Sirolimus are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ketoconazole inhibits CYP3A4 and P-glycoprotein, reducing sirolimus metabolism and efflux.

Avoid concurrent use and choose an alternative antifungal, such as an echinocandin. If unavoidable, reduce the sirolimus dose substantially and monitor trough levels closely.
