Per cosa si usa
Come agisce
Cefiderocol is a siderophore cephalosporin. It binds extracellular free iron and is actively transported into the periplasmic space of Gram-negative bacteria, then binds penicillin binding proteins, inhibiting peptidoglycan cell wall synthesis and causing cell lysis and death.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
cUTI and HABP/VABP (CLcr 60 to 119 mL/min): 2 grams IV infusion over 3 hours every 8 hours; duration 7 to 14 days guided by clinical status
Gravidanza e allattamento
- Gravidanza
- Safe No human data; animal studies showed no embryo-fetal toxicity. Cephalosporins as a class have not shown birth-defect risk. Use when needed for resistant infections with limited options.
- Allattamento
- Safe No human milk data; found in rat milk. LactMed: cephalosporins are not expected to harm breastfed infants, so it is acceptable; watch for loose stools or thrush.
Effetti indesiderati segnalati
- Diarrhea
- Infusion site reactions
- Constipation
- Rash
- Elevated liver tests
- Hypokalemia
Controindicazioni
- Known history of severe hypersensitivity to cefiderocol, other beta-lactam antibacterials, or any component of FETROJA
Chi deve prestare attenzione
- Increased all-cause mortality vs best available therapy in patients with carbapenem-resistant Gram-negative infections; monitor clinical response closely
- Serious and occasionally fatal hypersensitivity reactions; ask about prior beta-lactam allergy; discontinue if allergic reaction occurs
- Clostridioides difficile-associated diarrhea may range from mild diarrhea to fatal colitis
