Per cosa si usa
Come agisce
Collagenase digests collagen in the physiological pH and temperature range, removing detritus and contributing to granulation tissue formation and epithelization. Collagen in healthy tissue or new granulation tissue is not attacked.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Debridement of chronic dermal ulcers and severely burned areas: apply topically to the wound once daily, or more often if the dressing becomes soiled; stop when debridement is complete and granulation tissue is well established
Gravidanza e allattamento
- Gravidanza
- Unknown Injection (Xiaflex): blood levels are undetectable after injection; animal studies showed no harm; human case data are insufficient. Ointment (Santyl): not meaningfully absorbed. Use if needed; some labels advise postponing Xiaflex until after pregnancy.
- Allattamento
- Safe No milk data. Blood levels are undetectable after Xiaflex injection and the ointment is not meaningfully absorbed, so infant exposure is unlikely. US label: weigh benefits. Do not apply ointment to the breast.
Effetti indesiderati segnalati
- Transient erythema of surrounding tissue
- Systemic hypersensitivity (one case reported)
Controindicazioni
- Local or systemic hypersensitivity to collagenase
Chi deve prestare attenzione
- Enzyme activity is best at pH 6 to 8; higher or lower pH reduces activity.
- Detergents and heavy metal ions (mercury, silver) in some antiseptics reduce activity. Cleanse the site with repeated normal saline washings first. Avoid soaks containing metal ions or acidic solutions.
- Monitor debilitated patients closely for systemic bacterial infection, as debriding enzymes may theoretically increase the risk of bacteremia.
