Per cosa si usa
Come agisce
Humanized IgG2 kappa monoclonal antibody that binds P-selectin and blocks its interactions with ligands including PSGL-1; it can also dissociate preformed P-selectin/PSGL-1 complex.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Sickle cell disease (reduce frequency of vaso-occlusive crises): 5 mg/kg by intravenous infusion over 30 minutes at Week 0, Week 2, and every 4 weeks thereafter; may be given with or without hydroxyurea
Gravidanza e allattamento
- Gravidanza
- Unknown May cause fetal harm: in monkeys it increased fetal loss (abortions, stillbirths). Human data are insufficient. Sickle cell disease itself raises pregnancy risks. Use only if the benefit justifies the potential risk to the fetus.
- Allattamento
- Unknown No data on milk levels or infant effects. Human IgG passes into milk; effects of the small infant exposure are unknown. Weigh the benefits of breastfeeding against the mother's need for treatment.
Effetti indesiderati segnalati
- Headache
- Arthralgia
- Nausea
- Back pain
- Fatigue
- Abdominal pain
Chi deve prestare attenzione
- Infusion-related reactions (pain, nausea, vomiting, fatigue, dizziness, pruritus, diarrhea, pyrexia), some requiring hospitalization; mostly during first and second infusions. Interrupt or slow the infusion for mild/moderate reactions, discontinue for severe ones; consider premedication for later infusions.
- Infusion-related reactions can be indistinguishable from vaso-occlusive crisis events and may occur together.
- Use caution with corticosteroids in sickle cell disease unless clinically indicated; they may increase the risk of acute chest syndrome and fat embolism.
