Per cosa si usa
Come agisce
Nelarabine is a prodrug of ara-G, a nucleoside metabolic inhibitor. It is demethylated by ADA to ara-G and converted to the active triphosphate ara-GTP, which is incorporated into DNA, inhibiting DNA synthesis and causing cell death.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Relapsed or refractory T-ALL/T-LBL: 1,500 mg/m2 by intravenous infusion over 2 hours on Days 1, 3, and 5, repeated every 21 days; administer undiluted
Gravidanza e allattamento
- Gravidanza
- Unsafe Can cause fetal harm: it was teratogenic in rabbits at doses below the human dose; human data are too limited. Check pregnancy status before starting; women should use effective contraception during treatment and men should use condoms during and for 3 months after.
- Allattamento
- Unsafe No data on milk levels. e-lactancia: incompatible; a small, poorly protein-bound molecule likely to pass into milk. US label: do not breastfeed during treatment because of possible serious infant reactions, such as severe nerve toxicity.
Effetti indesiderati segnalati
- Anemia
- Thrombocytopenia
- Neutropenia
- Fatigue
- Nausea
- Cough
Avvertenze in evidenza
- Severe neurologic adverse reactions have occurred, including altered mental states with severe somnolence, convulsions, peripheral neuropathy from numbness and paresthesias to weakness and paralysis, demyelination, and ascending neuropathy similar to Guillain-Barré syndrome. Full recovery has not always occurred after stopping therapy. Monitor frequently for neurologic toxicity and discontinue for CTCAE Grade 2 or greater neurologic reactions.
Chi deve prestare attenzione
- Neurologic toxicity: monitor during and for at least 24 hours after treatment. Prior or concurrent intrathecal chemotherapy or prior craniospinal irradiation may increase risk.
- Hematologic toxicity: leukopenia, thrombocytopenia, anemia and neutropenia; monitor complete blood counts including platelets regularly.
- Embryo-fetal toxicity: can cause fetal harm; verify pregnancy status, use effective contraception, and males should use condoms during treatment and for 3 months after the last dose.
