Per cosa si usa
Come agisce
Oliceridine is a full opioid agonist, relatively selective for the mu-opioid receptor; its principal therapeutic action is analgesia, with no ceiling effect to analgesia.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Acute pain severe enough to require an IV opioid: initial dose 1.5 mg IV given by a healthcare provider; supplemental doses of 0.75 mg beginning 1 hour after the initial dose and hourly thereafter as needed; single doses greater than 3 mg not to be given; maximum cumulative daily dose 27 mg.
Gravidanza e allattamento
- Gravidanza
- Unsafe Prolonged use in pregnancy may cause neonatal opioid withdrawal syndrome; no human data on birth defects. Animal studies showed reduced live litter size and increased pup mortality. Not recommended during and immediately before labor.
- Allattamento
- Unknown It is not known whether oliceridine is in human milk; weigh the benefits of breastfeeding against the mother's need. Monitor exposed infants for excess sedation and respiratory depression.
Effetti indesiderati segnalati
- Nausea
- Vomiting
- Dizziness
- Headache
- Constipation
- Pruritus
Avvertenze in evidenza
- Exposes users to risks of opioid addiction, abuse and misuse, which can lead to overdose and death; assess risk before prescribing and reassess regularly. Serious, life-threatening or fatal respiratory depression may occur, especially during initiation or after a dosage increase. Concomitant use with benzodiazepines or other CNS depressants, including alcohol, may cause profound sedation, respiratory depression, coma and death; reserve for patients with inadequate alternatives. Prolonged use in pregnancy can cause neonatal opioid withdrawal syndrome, which may be life-threatening.
Controindicazioni
- Significant respiratory depression
- Acute or severe bronchial asthma in an unmonitored setting or without resuscitative equipment
- Known or suspected gastrointestinal obstruction, including paralytic ileus
Chi deve prestare attenzione
- QTc prolongation risk with cumulative daily doses above 27 mg; do not exceed 27 mg.
- Increased oliceridine exposure with decreased CYP2D6 function or with CYP3A4 inhibitors (or stopping CYP3A4 inducers) can prolong adverse reactions and worsen respiratory depression.
- Opioid-induced hyperalgesia and allodynia may occur; consider dose reduction or opioid rotation if suspected.
