Come agisce
Recombinant PEG-conjugated human G-CSF; acts on hematopoietic cells by binding to specific cell surface receptors, thereby stimulating proliferation, differentiation, commitment, and end-cell functional activation

Prevention of Chemotherapy-induced Neutropenia: 6 mg SC once per chemotherapy cycle

- Unknown Limited pegfilgrastim data; filgrastim studies show no link to birth defects or miscarriage. No harm in rats at 10 times the dose; rabbit fetal loss only with maternal toxicity. EU label: not recommended in pregnancy or without contraception. Filgrastim has more data; use only if clearly needed.

- Safe No milk data for pegfilgrastim. EU label: risk cannot be excluded; decide whether to stop breastfeeding or the drug. LactMed (filgrastim record, covers pegfilgrastim): G-CSFs pass poorly into milk and are not absorbed by the infant's gut, so harm is unlikely; some advise pausing feeds for 3 days.

- Severe and sometimes fatal sickle cell crises can occur in patients with sickle cell disorders; discontinue therapy if sickle cell crisis occurs
- ARDS reported; evaluate symptoms, including fever, pulmonary infiltrates, or respiratory distress; discontinue if ARDS occurs
- Rare cases of splenic rupture may occur; evaluate if patient experiences left upper abdominal and/or shoulder tip pain
