Come agisce
Selpercatinib is an inhibitor of the RET receptor tyrosine kinase. It also inhibited VEGFR1 and VEGFR3 and, at higher concentrations, FGFR 1, 2 and 3.

RET fusion-positive/RET-mutant tumours (patients 12 years and older, under 50 kg): 120 mg orally twice daily until disease progression or unacceptable toxicity

- Unsafe Can cause fetal harm: in rats it caused embryo death and malformations at about human exposure. No human data. EU label: not recommended in pregnancy or without contraception. Check pregnancy status; use highly effective contraception during and for at least 1 week after.
- Allattamento
- Unsafe No human milk data. LactMed: 97% protein bound, so milk levels are likely low. Because of possible serious infant reactions, US and EU labels say do not breastfeed during treatment and for 1 week after the last dose.

Controindicazioni
- Hypersensitivity to the active substance or to any excipient.
Chi deve prestare attenzione
- Severe, life-threatening or fatal ILD/pneumonitis; withhold, investigate and adjust or discontinue.
- Increased ALT/AST: monitor before treatment, every 2 weeks for 3 months, then monthly for 3 months.
- Hypertension: control blood pressure before starting and monitor; discontinue permanently if it cannot be controlled.
