Sintesi
Acemetacin, an NSAID, can reduce renal prostaglandins and impair potassium excretion, adding to amiloride's potassium retention. It may also reduce the diuretic and antihypertensive effects and raise the risk of acute kidney injury.
Cosa significa questa interazione in parole semplici
Acemetacin and Amiloride can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Perché avviene
NSAID-induced suppression of renin-aldosterone and renal prostaglandins reduces potassium and sodium excretion, combined with amiloride's ENaC blockade.
Cosa fare
Avoid the combination where possible, especially in CKD or the elderly. If needed, monitor potassium, creatinine and blood pressure closely.
Cosa chiedere al farmacista
