Rezumat
Methylene blue inhibits MAO-A and can precipitate serotonin syndrome when given with desvenlafaxine. The reaction can be severe, with hyperthermia, neuromuscular hyperactivity, and altered mental status.

Desvenlafaxine and Methylene blue are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

MAO-A inhibition by methylene blue adds to serotonin and norepinephrine reuptake inhibition, causing excess serotonergic activity.

Avoid the combination. If methylene blue is urgent, stop desvenlafaxine and monitor for serotonin syndrome for up to 2 weeks or until 24 hours after the last methylene blue dose, whichever comes first.
