Для чего применяется
Как действует
Stimulator of soluble guanylate cyclase (sGC). It sensitizes sGC to endogenous nitric oxide and also directly stimulates sGC independently of NO, increasing cGMP and causing vasodilation.
Обычная доза для взрослых (кратко по инструкции)
CTEPH or PAH: start 1 mg orally three times a day (consider 0.5 mg three times a day if hypotension may not be tolerated); if systolic BP stays above 95 mmHg with no hypotension symptoms, increase by 0.5 mg three times a day no sooner than every 2 weeks; maximum 2.5 mg three times a day

- Unsafe Contraindicated in pregnancy; animal studies show fetal harm including birth defects, fetal death and miscarriage. Human data are limited.

- Unsafe No human milk data; present in rat milk. Advise not to breastfeed because of risk of hypotension in infants.

- Embryo-fetal toxicity: contraindicated in pregnancy because it may cause fetal harm based on animal data. Exclude pregnancy before treatment. Use effective contraception before, during and for one month after treatment. If pregnancy is detected, discontinue as soon as possible.

- Pregnancy
- Concomitant nitrates or nitric oxide donors in any form
- Concomitant PDE inhibitors (specific PDE-5 inhibitors and nonspecific such as dipyridamole or theophylline)

- Hypotension: reduces blood pressure; consider risk in hypovolemia, severe left ventricular outflow obstruction, resting hypotension, autonomic dysfunction, antihypertensives or strong CYP/P-gp/BCRP inhibitors; consider dose reduction.
- Bleeding: serious bleeding occurred in 2.4% of patients on Adempas vs 0% placebo, including serious hemoptysis in 1%.
- Pulmonary veno-occlusive disease: not recommended; if pulmonary edema occurs, consider PVOD and discontinue if confirmed.
