Gentamicin can cause renal tubular damage with magnesium and potassium wasting, and hydrochlorothiazide adds to potassium and magnesium loss. Diuretic-induced volume depletion also raises the risk of aminoglycoside nephrotoxicity and ototoxicity, while valsartan can worsen renal function in volume depletion.
