Drug interactions
Acetylsalicylic acid + Omega-3-acid ethyl esters
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive antiplatelet effects may increase bleeding risk (e.g., bruising, epistaxis, gastrointestinal bleeding) and prolong bleeding time, particularly at higher omega-3 doses.
Mechanism of action
Pharmacodynamic: EPA/DHA incorporation into platelet membranes reduces thromboxane A2–mediated aggregation; aspirin irreversibly inhibits COX-1, decreasing thromboxane A2—resulting in additive inhibition of platelet function.
Clinical management
Use the lowest effective aspirin dose and monitor for bleeding (bruising, epistaxis, melena, hematuria). Exercise additional caution at omega-3 doses ≥3–4 g/day of EPA+DHA. Consider holding high-dose omega-3 prior to elective procedures after risk–benefit discussion.
