Summary

NSAIDs such as parecoxib can reduce renal prostaglandin-dependent perfusion and lower aminoglycoside clearance. This can raise amikacin concentrations and the risk of nephrotoxicity and ototoxicity, particularly in neonates and in patients with renal impairment.

What this interaction means in plain words

Amikacin and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Reduced renal blood flow and glomerular filtration from cyclooxygenase-2 inhibition decreases renal elimination of amikacin.

What to do

Monitor serum creatinine and amikacin trough levels, and adjust the amikacin dose accordingly. In preterm infants or patients with renal impairment, avoid the combination where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
