Summary

NSAIDs can blunt the renal effects of potassium-sparing diuretics and raise serum potassium. Hyperkalemia and acute kidney injury are more likely in older patients or those with renal impairment or diabetes.

What this interaction means in plain words

Amiloride and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiloride belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAIDs suppress prostaglandin-mediated renin and aldosterone release and reduce renal blood flow, impairing potassium excretion that is further reduced by amiloride.

What to do

Check serum potassium and creatinine within the first weeks and periodically thereafter. Avoid in patients with renal impairment or other hyperkalemia risks.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
