For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Vandetanib inhibits the renal organic cation transporter OCT2, which is involved in tubular secretion of amiloride. Amiloride exposure may therefore rise, increasing the risk of hyperkalemia. Vandetanib also prolongs the QT interval, and electrolyte disturbances worsen that risk.

What this interaction means in plain words

Amiloride and Vandetanib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiloride belongs to Cardiovascular; Vandetanib belongs to Oncology.

Evidence level: Low

Why it happens

Vandetanib inhibits OCT2-mediated renal tubular secretion of amiloride, reducing its clearance.

What to do

Monitor serum potassium and ECG/QTc regularly during co-therapy. Correct potassium abnormalities promptly and reduce amiloride dose if hyperkalemia develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
