Summary

Palonosetron is a 5-HT3 antagonist, and serotonin syndrome has been reported when such agents are combined with serotonergic antidepressants. Netupitant inhibits CYP3A4 and may modestly increase amitriptyline exposure. The risk is low with short antiemetic courses.

What this interaction means in plain words

Amitriptyline and Netupitant / palonosetron can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Possible additive serotonergic effects, plus netupitant inhibition of CYP3A4 that may raise amitriptyline levels.

What to do

Monitor for agitation, tremor, hyperthermia and other signs of serotonin toxicity, as well as for anticholinergic effects and QT prolongation. Treat any suspected serotonin syndrome by stopping the serotonergic agents.

What to ask your pharmacist
