Summary

Analgesic doses of aspirin can reduce the antihypertensive effect of ACE inhibitors by inhibiting vasodilating prostaglandins. Together they can also impair renal function, particularly in elderly or volume-depleted patients, and the sodium load from bicarbonate may add to this.

What this interaction means in plain words

Amlodipine-benazepril and Aspirin / citric acid / sodium bicarbonate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Cyclooxygenase inhibition reduces renal prostaglandin synthesis, causing sodium retention and reduced glomerular perfusion that counteract ACE inhibitor effects.

What to do

Limit use of analgesic-dose aspirin products; monitor blood pressure and renal function, and ensure adequate hydration. Low-dose aspirin for cardiovascular prevention does not usually need adjustment.

What to ask your pharmacist
