Summary
Cyclosporine can raise amlodipine exposure, and amlodipine may modestly raise cyclosporine concentrations. Benazepril combined with cyclosporine adds risk of hyperkalemia and reduced kidney function, particularly in transplant recipients.
What this interaction means in plain words
Amlodipine-benazepril and Cyclosporine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
CYP3A4 and P-glycoprotein-related pharmacokinetic effects, plus additive renal hemodynamic effects and potassium retention from ACE inhibition with cyclosporine.
What to do
Monitor cyclosporine trough levels, serum creatinine and potassium when starting or stopping the combination. Adjust the cyclosporine dose as needed and reduce amlodipine if hypotension or edema occurs.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
