Summary
Ibuprofen can reduce the antihypertensive effect of olmesartan. With an ARB, NSAIDs can also precipitate acute kidney injury and hyperkalemia, especially in elderly or volume-depleted patients.
What this interaction means in plain words
Amlodipine-olmesartan and Ibuprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing ARB effects.
What to do
Monitor blood pressure, creatinine and potassium, and keep the patient hydrated. Use the lowest effective ibuprofen dose for the shortest time or choose paracetamol.
What to ask your pharmacist
