What this interaction means in plain words

Amlodipine-olmesartan and Spironolactone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-olmesartan belongs to Cardiovascular; Spironolactone belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive suppression of the renin-angiotensin-aldosterone system effect on renal potassium excretion.

What to do

Check serum potassium and creatinine before starting and within the first week or two, then at regular intervals. Reduce or stop spironolactone if potassium rises above the accepted range, and avoid potassium supplements.
