Summary
Carbenoxolone has mineralocorticoid-like activity that causes sodium retention, hypokalemia and hypertension, which can oppose the antihypertensive effect of the combination. Its effect on potassium partly offsets valsartan but the net result is unpredictable.
What this interaction means in plain words
Amlodipine-valsartan and Carbenoxolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Carbenoxolone inhibits 11-beta-hydroxysteroid dehydrogenase, causing mineralocorticoid excess, sodium and fluid retention and potassium loss, counteracting ARB and amlodipine effects.
What to do
Monitor blood pressure, serum potassium and fluid status. Consider an alternative agent if blood pressure control deteriorates.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
