Summary
Ketorolac can reduce the antihypertensive effect of valsartan and, together with an angiotensin receptor blocker, raises the risk of acute kidney injury and hyperkalemia. Ketorolac is itself nephrotoxic and the risk is greater in volume-depleted or elderly patients.
What this interaction means in plain words
Amlodipine-valsartan and Ketorolac can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-valsartan belongs to Cardiovascular; Ketorolac belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandin synthesis by ketorolac reduces glomerular perfusion, which is made worse by angiotensin receptor blockade, and impairs potassium excretion.
What to do
Avoid or limit ketorolac to a very short course with adequate hydration. Monitor serum creatinine, potassium and blood pressure.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
