Summary
Ritonavir-boosted regimens strongly inhibit CYP3A4 and raise amlodipine exposure, increasing the risk of hypotension and peripheral edema. Paritaprevir and ritonavir may also inhibit the hepatic transporters that handle valsartan, which could modestly raise its levels.
What this interaction means in plain words
Amlodipine-valsartan and Ombitasvir / paritaprevir / ritonavir and dasabuvir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of CYP3A4 by ritonavir and paritaprevir raises amlodipine levels, and inhibition of hepatic uptake transporters (OATP1B1/1B3) may raise valsartan levels.
What to do
Reduce the amlodipine dose by at least half and monitor for hypotension and edema, reducing further or switching if these occur. Monitor blood pressure and renal function for valsartan effects.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
